Harlequin Crest is a Mythic Unique available exclusively in the Torment difficulty tiers. Drop rates improve as you progress into higher Torment levels.
While any monster can drop Harlequin Crest, Belial offers the highest drop rates by far. Andariel and Duriel come next, with 50% better odds than other Lair Bosses.
