Royal Sapphires are the secoond highest quality of Sapphires. There are 5 qualities in total and information on all of them can be found on the Sapphire guide.

The Gems guide details all the different kinds of gems that can be socketed into gear.

Critical Strike Damage to Crowd Controlled enemies removed.
Damage Reduction While Fortified removed and replaced with +100 Willpower.
Jewelry Cold Resistance increased from 30% to 45%.
Cost increased from 1,350 Sapphire Fragments to 10,000 and 50,000 Gold to 1,000,000.
