'Grants Player' means Grants Exploits creates this condition i.e it can make a target Vulnerable. 'Exploits Condition' means Grants Exploits takes advantage of this condition, not necessarily of its own making. So for example another skill could Stun and Grants Exploits can take advantage of that.
