After you have finished the Rite of Passage quest, and continued in from the Arreat Summit in to the Keep, you'll now start to meet the most elite troops Hell has got to offer. Many are from Baal's personal guards, and they will do anything to keep you from the Throne of Destruction.

The Keep consists of three levels of monsters, before you reach the Throne, and on Level 2, there is a waypoint for quicker access.

This is the only level in all of Act V that has multiple levels with the same name, rather than Halls of Pain, Halls of Vaught etc.
