You find the Crystalline Passage located after Arreat Plateau. Inside you will find two other exits leading to the Frozen River (the Prison of Ice quest), the other leading further and into the Glacial Trail.
There is a waypoint somewhere in this level, and it's a good idea to pick it up if you are passing by.
A small warning for the Glacial Trail here, since in the story line, you normally have to go through some more levels to kill Nihlathak, and would otherwise possibly have leveled up before entering. Continue without doing Nihlathak on your own life!
