Ber and Vex (both from Mephisto), and Ist (and everything below) from the Countess.

Several Ber's, Vex's, Mal's and Ist's. Mostly in Pit runs but some have come from the Chaos Sanctuary. I even found a Lo rune once from a chest at the entrance of Durance 3 but sadly it was a Non-Ladder game. The highest I've ever received from The Countess was a Pul.

Lo rune , got it from chest in the area after LK i forgot the name. I have also found several ist runes - 1 from cows, 1 from random monster in act 5 , one from countess, one from baal run. All this in less than 3 months playing

Crowd Control: Likely. (Definitely in the case of lhtb and Low Key.)
For me, only a Lo from Chaos Sanctuary, and a Vex from chest in Catacombs 2..

Gul from the cow level. Never seen a rune higher than what HF can give drop in over eight years of playing.

Over a period of 3 seasons:
Pul from a rock in Cows!
Um from a lightning beetle or maggot in the Maggot Lair
Mal from Countess
2xIst from Countess
Gul from Countess
Vex from a chest at the end of one arm of the arcane sanctuary
Ohm from WSK level 2 from a random spawn
If you run a lot of hell countess you will get a good collection b/w Hel-Ist from her Rune drop and if you are really lucky from her Normal drop you can get up to Lo.
Over a period of 3 seasons:

Vex, Hell travincal (took me 3 weeks, running 10 hours per day iirc)
I was looking for a fast and easy area, where runes drops more frequently than other places, and I found it ^^
I did made another Vex from 2 Ist and 1 Gul (all hellforge runes)
