Nightmare Meph cannot drop shakos, but you don't need one. There are more important items to gather before.
My sorc has a Peasant Crown, Spirit sword, Lidless+pdiamond, Stealth, Magefists, a rare belt, rare boots, and ringss and amu with resists and enough FCR for the 105% FCR breakpoint (Gull+Rhyme in the switch, Wealth, 3 ptopaz helm and Frosties in the stash, for more effective MF). I respecced her into a meteorb and I just finished hell act 1 with her, solo. I left out the Countess quest, however, she's cold+fire immune and my merc is probably too weak for her (respectively, I was too lazy and didn't want to try it). I simply teleported through all of it and I guess I will do the same in act 2 and 3. The next serious obstacle might be the councils in Travincal, one of them is always cold+fire immune as well. AFAIK it's not the one who drops the flail, however.
