MFO When and Format?

I think it would be 94 because normal monsters get +2 mlvl and it caps out at 96.

I think the tc "upgrades" (change in tc from desecrated A to B, B to C and so forth) occur every 3 levels, however i have no idea where i remember this from. There is most certainly another tc upgrade between @Vildecor 's 90 and the cap which likely means the drop distribution is even more skewed.
 
Yes, character level 91 will push uniques up to monster level 96, which allows the highest terrorized drop tables for most zones. I had calculated the changes for character level 99 for the Unholy corspes in WSK 1 and didn't see too much difference, but here is the data:

Basically, all uniques, including TC 87's increase their rarity compared to the IKSC by about 1%, with the class-specifics becoming slightly more rare at 2.8%. This is not much difference, which should be expected since this is for level 85 area, which already was experiencing the TC87 double dip in LOD according to the previously linked video. The biggest difference is when champions and regular monsters hit a high enough level to allow them to also double dip. This change along with the changes to superuniques and act bosses is the reason why we are seeing more TC87s drop, since previously only select areas/monsters could drop anything in the game.

My contention is that for the MFO running terrorized areas, the drop chances for elite uniques do not differ enough to warrant changes to their point values. The relative rarities are very similar when compared to those for regular level 85 areas in LOD. Even if we include possible drops from champions and regular monsters, those relative rarities will be very similar as well (1.6% increase for TC87s from regular monsters and 0.8% for champions at character level 99).
