Keys are dropped randomly, I think. You get one from the Countess in the Tower in the Black Marsh (act1), one from the Summoner in Arcane Sanctuary (act2), and the third in the Halls of Pain from Nihlithak (act5).
The only places to find these keys...
Hell Countess drops the Terror Key, Hell Summoner drops the Hate Key, and Hell Nihl drops the Destruction Key.
The odds are approximately 1:12 for H and T, and 1:13 for the D key. I say approximately, because you may find that your mileage varies significantly, if you don't do enough runs to obtain a statistical accuracy. But everyone pretty much agrees, it's all going to average out to about a 1:12 chance for any given key - in the long run.
