I had Mated my 1.13beta Kicker hybrid, thread pending, and she was fairly close to going up a level so I thought kill the Cow King and do a few Baal runs. The Cow King dropped zilch, so I decided to visit Baal for a while. I didn't even get to him once before this dropped:
