nihlathak quest bug?

Hail,

I have been running a few D key runs with my hammerdin and this question arose. I never seem to complete the quest whenever I kill him. What's the deal?

I am in bnet, hell difficulty, in a private game by myself. My quest tab shows that I do not have the quest, nor have I completed it. I take the halls of Pain WP. Upon entering the halls of pain via the WP, I get the red quest icon on my screen.. but if I click it, it appears that I still do not have the quest, nor have I completed it. So I run down to halls of vaught.. and once I get about 1-2 screens away from Nihlathak, I get the red quest icon again. This time I open the quest screen and I have the quest, and it simply says "Kill Nihlathak",, Ok so I proceed to knock off some of his minnions before killing him.
Upon killing him, a red quest icon appears. I open the quest screen and it says "You cannot complete this quest in this game, trying entering another game or start a new game" type message.

This same scenario happens every single game no matter what over the 40 or so D key runs I have done.. What is happening here?

I'm not sure but maybe the quest must start with Anya.
Maybe it's that, did someone just gave you the wp for nil so you never spoke to Anya to get the portal up?

I spoke with anya before to claim the item reward for saving her. I am not sure if I ever went through the portal on this char. A friend gave me the WP.. and since I have the WP and have killed nihlathak.. Anya does not spawn nor does her portal anymore.

But, I have still not completed the quest.

Anya quest has you personalize an item once you kill nihlathak to finish the quest.
So if anya doesn't spawn then your not getting your quest .(i Think)

try looking for her again in crystalline passage just to check if she is there.

If not when no big lost. (unless if you want all quest done XD)
