Belt with Prevent Monster Heal

I just found Hwain's belt which has prevent monster heal. How does this work in a belt? Do I need to hit the monster still to prevent the heal, or is it essentially an area effect? What if one of my minions (I'm a fishymance with no hopes of landing a hit in Hell) hit the monster, will it carry over?

I'd like to keep the belt, even with only 12 spots, if the PMH works for me. IF not, that I'll roll back to my battle bell with high RES.

If you're wearing the belt, only attacks made by you will carry over the PMH affect over to enemies. If your merc/minions make the attack, the PMH won't do anything.
 
And iirc, only physical damage will result in triggering "prevent monster heal" "open wounds" "critical strike" "deadly strike" "crushing blow"

So you'd have to hit manually, no poison nova and no bones attacks.
