Pretty much as the title of the thread says...

Using a phase blade for the socket reward quest from act 5 how many sockets would I get in a phase blade?

The socket quest gives the maximum number of sockets possible. And that is limited by the item level. You can check this link for the max number of sockets by levels.

Not to hijack the thread, but I have a question:

The item level is equal to the level of the monster that dropped that item, right?

Unless you've used a cube recipe to turn a low quality phase blade into a normal quality one, it'll get six sockets (assuming its white).
