Grief works very well, you need 42% IAS with a PB to reach max 2.77 berzerk attacks per second so any low IAS gear item will give you max attack speed with a decent IAS grief.
Grief is also very good to use because of the low str requirement, and if you use a light or medium armor/shield rather than heavy you can put those extra points into vitality rather than strength as you really want at least 2k hp with BO by level 80 but preferably more (especially if you don't use a shield).
As for your damage what is your AR? If you are not one shotting most mobs with a grief pb (max zerk, Howl, shout, and working on Mastery) in hell then I would be looking at your AR in the character screen, is it always at 95% chance to hit? If not then that's your problem. (Maybe max mastery earlier).
I have been using a 'Shael'd Whitstan's Guard (60%) for 4 frame blocking, but a Shael'd SS (55%) would also reach 4 frame blocking (42% for barb, 86% for 3 frame).
I use an 'Um' Arreats, Tal's Armor, Gore Riders, Highlords, 2* Ravenfrost or 1 RF ring and 1 Nature's Peace, Verdungo's, and currently Chance Guards.
Lghtning and Cold don't bother me, poison from bosses hurts, and I can take a bit of fire.
1 point in dbl swing and a light saber or [insert favourite weapon here] in the off hand is another way to do it if you prefer to clear the mobs as you go.
Also forget leech, try the Prayer Merc (act 2 combat skills) with an insight polearm and don't forget pots.
