No its not hacked. It dropped in Diablo Classic in one of the early patches (1.07 or earlier) The item is duplicated and all stay prem in the game.
I think there are 4 or 5 of these helmets still in the game.

that's pretty cool, versatile helm there. i usually keep heart carver in my barbs' stash for when i want to FI a bit, but that one covers it better what with the bo prebuffing availability as well.

Just found this tonight. maybe 2nd best yellow weapon I ever had (search this thread for duckbutter's imp saw). This one is too slow for ww barb, but still perfect for off-hand frenzy, berserker, or avenger. Imagine the damage if it was ethereal...
