I think that a prequel will just smell like more of the same. Yeah, new locations and characters; but basically everyone is still going to be waving axes and casting spells and clicking mouses.
The best direction for the series to move in, in my opinion, is into the future. Imagine the darkness of tristram and kurast and add eight hundred years. Abandoned spaceships from unknown solar systems, with dark, blood stained corridors. Crawling with all sorts of demonic and alien creatures.
And more importantly, you'd get different sorts of weapons beside the axe/bow variety.
As for gameplay interface, moving away from the point and click interface might make things fresher. First person is always an option, with inventory and item grabbing working like system shock 2, with a pull down box into which you can toss items with a point and click of the reticle. Or maybe make the interface like 'hunter: the reckoning.' Still 3/4 action, but aiming is done more generally rather than specifically.
