About the "Magic Item + 3 Pgems = New magic item" recipe: Can I roll a rare jewel? Or a Unique Charm/Weapon/Armor? Or only normal magic items?

Can you reroll rare charms? Yes, with six perfect skulls. Please note the level of the item will drop. Magic (blue) items are the only ones you can re-roll without decreasing the item's level.

You cannot re-roll unique or set items.

Edit: Additional info on re-rolling rares, and the impact to the item's level - there are two recipes, although I am unaware of anyone who has enough Stones of Jordan lying around to use the first:

1 perfect skull, 1 Stone of Jordan, 1 rare item = 1 rare item; ilvl = 0.6 ilvl + 0.6 clvl
6 perfect skulls, 1 rare item = 1 rare item; ilvl = 0.4ilvl +0.4clvl

No , I meant using a simple magic weapon/charm/jewel with the recipe and getting a rare/unique weapon/charm/jewel.

I think this is a typo There is no such thing as rare charms. I'm sure Noodle meant rare Jewels here.

No , I meant using a simple magic weapon/charm/jewel with the recipe and getting a rare/unique weapon/charm/jewel.

You can take a magical item (certain armor items, certain weapon types, ring, amulet) and add a jewel, a certain perfect gem and a certain rune and cube those to make a crafted item. These items have orange text and they have 3-4 predetermined abilities (their values can vary) and 1-4 random abilities, taken from same affix pool as for rare items. See here for more.

In prior versions (.07?) I think you could cube set items. I think I've seen a Tal's Ammy that was made in the cube. Had extra cold resists perhaps?

Indeed. You could cube set/rares/uniques. Doens't work now anymore though.
