Either one works. However I recommend the first. The reason for casting BC twice is so as to get the extra skill point on the BC to extend it's time. By that strategy using the BC, Shout, BO, BC gives an extra second or so of the extra skill point over BC, BC, Shout, BO

Ok this isn't hal's setup but it is mine, so here goes. I use ERTY as belt keys, and keep my ring index pointer fingers on ERT, I set the display key (alt) to z and that is where my pinky goes, thumb on space. Skill keys are 2345678SDFGHXCVB. Map is A. 3 is Town portal 4 is Howl 5 is taunt 6 is bcry 7 is find potion 8 is find item. W is weap switch, S is zerk, D is conc, F is throw, G is Leap attack, H is Leap, X is Grim ward, C is BC, V is BO, B is Shout. Other keys of interest, Shift is Stand still, CTRL toggles run/walk, I is still inventory. Other keys like party hireling etc are set up on the far right of the keyboard out of the way. To the later part of that question i set conc or zerk to my left, depending on situation, and the rest all go on right depending on situation again. I never use bash.

You get very very lucky, do millions of countess and hellforge runs and cube, or you trade for one of the duped ones. I think berserker axes are good chance through the portal in the frigid highlands. But hal will confirm that.

I'd recommend using dex charms to get your dex up temporarily. Also if you don't have the endgame gear waiting you may have to put a few more points into dex and str then would be prefered, but it's still a playable build without perfect stat placement. My barb has way more str and dex then he needs so his vit is only like 3000, I started him blank at ladder start so i had to lay out stats for gear available, yet he does fine.

Not sure what you mean, if you mean will this barb be able to find his own gear or is a mf sorc necessary, then this barb can do it, but a mf sorc would speed up the process, My barb found most of his own gear, and all i'm missing is botd, and enigma.
