There is a well known glitch that crashes the game when you transfer between acts. It seems to happen whenever the game tries to display the little cut-scene between acts.
To fix it, you can either play in windowed mode (ie not full-screen), or you can disable the sounds. Either way should do it. I had this problem for ages, and this combonation of things fixed it. Hope this helps.
