I'm guessing that Thrugg has the exact formula for this... I'm at work right now so I don't have it available but Thrugg explained it too me in the Stats forum a few months ago if you want to go search through that, I have it written down at home.
The required level is not only based on WHAT mods are on an item, but HOW MANY mods come on an item. It isn't a matter of having 6 mods and the highest one is lvl 64 - so the required level becomes 64. The combination of mods SCALES UP - and scales up higher on crafted items than on rare items. If a crafted item and rare had the EXACT same mods, the required level on the crafted would always be higher. So, a dual leech of say 3% life, 3% mana by itself wouldn't be all that high. But a 7% / 7% with 4 other mods (especially plus any skills) would be much higher.
I know that dual leech tends to jack up the required levels. One of my favorite crafts are Safety Amulets - I try to get +2 All Skills with 10% Chance to Block. If I do happen to get that combination with dual leech the required level very often goes up over 96.
This one doesn't have + skills but it is a good example of what happens when a crafted has high resist all with just mediocre dual leech:
